By completing and submitting this form I authorize staff of the University Libraries to check out materials to me using the CWID number which I have placed on file with the UA Libraries.

I also agree to adhere to the policies and procedures governing the Allegro Courier Service.

I understand that I must be a valid Libraries user and have a CWID number on file in the Gorgas Music Library in order to use the Allegro Courier Service.


Allegro Courier Service Policies and Procedures

  • All Libraries Circulation Dept. policies apply to items delivered via the Allegro Courier Service.
  • Before items can be checked out to an individual for courier delivery, an authorization proxy form for that person, containing a valid CWID number, must be on file in the Gorgas Music Library.
  • Any circulating materials or journal articles from Gorgas Library or the Libraries Annex may be requested for courier delivery to the School of Music.
  • Large orders may be divided into sequential deliveries if space is not available on the Libraries mail delivery van for the entire order.
  • Requested articles will be scanned and emailed to patrons.
  • Deliveries and pickups will be made twice daily on Monday through Friday, when classes are in session, between the hours of 9:00-11:00 a.m. and 1:00-3:00 p.m.
  • Typically, an item that is available in Gorgas Library and requested by 10:00 a.m. Monday through Friday will be delivered the same afternoon. An available item requested by 2:00 p.m. Monday through Thursday will be delivered the following morning. The anticipated maximum turnaround time for delivery of items from the Annex will be 48 hours from time of request, except on Friday, when delivery will be the following Monday.
  • All materials will be delivered to a designated area in room 175 Moody Music Building. A copy of the request form will be attached to each item.
  • An individual authorized by the School of Music must sign for the items when they are delivered. Materials which are ready for pickup will be placed by the borrower in a designated area of room 175 Moody Music Building, preferably with the request form attached.
  • After the materials are picked up, they will be returned to the circulation desk in the Music Library where they will be checked in. Receipts will be printed for each item and sent to the individual patron through campus mail. If materials are returned by any means other than courier delivery, a receipt will only be printed if requested in person at the Gorgas or Music Library circulation desk at the time of check-in. Delivery receipts cannot be printed after the fact.
