Procedures For Graduate Assistants Placing ILL Requests As A Proxy For Faculty

Fill out proxy form online at: http://www.lib.ua.edu/circulation/ProxyForm.htm [1].
Log into ILLiad at: https://ua.illiad.oclc.org/illiad/ [2] using your MyBama username and password.
Create a new ILLiad account if you do not have one already.
Place request for book or article.
Put the name of the Faculty ordering for in the note field.
The requests placed will be in the GA’s name. The history of all request ordered by the GA for Faculty will be on the GA‘s account.
Articles will be delivered to the GA’s e-mail address. When an article arrives, it will need to be forwarded to the Faculty member.
Books will be available for pick-up at the Circulation Desk on the 2^nd floor of Gorgas Library all hours the library is open.
A valid Action Card is needed to pick up ILL materials.
Please contact the ILL Supervisor in the event the ILL material is lost or damaged while in the possession of the Faculty member.
