Proxy Form for Checking Out Materials

Faculty members can authorize another person as a proxy borrower to check out materials on the faculty member’s account by using this form. A proxy borrower may be a student, a staff person or an immediate family member. The faculty member should complete the form, and the proxy borrower should bring it to any of the University Libraries (McLure, Bruno, Rodgers or Gorgas) so the information can be added to the Libraries' computer system. The proxy borrower should then present their own Action card as well as the form whenever they check out materials for the faculty.
